Senior Health
Our goal is to give clients the knowledge
and tools to achieve optimal health so that they can continue to live
independently. We offer the following services:
- Talk With the Nurse
One-on-one visits with a public health nurse to discuss health issues (medication, resources, etc). Blood pressure checks are also available. Held monthly at Creekside Community Center. No appointment necessary.
- Senior health tips
Tips on how to attend to changing health needs.
- Home visits to the elderly
Public health nurses assess physical, psychosocial and environmental risk factors and provide referrals, consultation, education and advocacy. Referrals from any concerned community members are accepted. There is no fee for this service. Call 952-563-8900 to make a referral.
- Hearing screening for seniors
- At Creekside Community Center
1-3 p.m., the second Wednesday of even months
