City processes passport applications

The City Clerk's Office at Bloomington Civic Plaza accepts passport applications for processing.


What is the difference between the U.S. passport book and the U.S. passport card?

U.S. passport book: U.S. citizens may present a valid U.S. passport book when traveling via air, land or sea between the U.S. and other countries.

U.S. passport card: U.S. citizens may present a valid U.S. passport card on a limited-use basis. The passport card will be valid ONLY for land and sea travel between the U.S. and Canada, Mexico and the Caribbean region (including Bermuda).

What documentation is required when applying for a passport?

 All first time applicants must apply in person, submit a completed passport application, provide required identification, proof of U.S. citizenship, two identical passport photos, and pay the necessary fees.

Individuals who have had a previous passport, may be able to apply for a renewal passport by mail if they:

1) Can submit most recent U.S. passport,

2) Were at least 16 years old when most recent passport was issued,

3) Were issued most recent U.S. passport less than 15 years ago, and

4) Use the same name as on most recent U.S. passport or have had name changed by marriage or court order and can submit proper documentation.

Acceptable forms of identification include a driver’s license (not temporary or learner’s permit), a previous U.S. passport, a Certificate of Naturalization, or an official military identification card.

Acceptable proof of U.S. citizenship includes:

1) Certified copy of birth certificate,

2) Original naturalization certificate, or

3) Most recent U.S. passport.

If individual has been issued a previous U.S. passport, the passport must be provided at time of application.

What documentation is required when applying for a child’s passport?

Children under the age of 16, and their parents, must be present when the child’s application is presented for processing. Consent from both parents is required. Children between the ages of 16 and 17 also need parental approval for the application to be processed, but both parents are not required to give consent. Required documentation includes:

1) Child’s identification, if child is 16 to 17 years of age and s/he has photo identification;

2) Previous passport, if applicable;

3) Certified copy of birth certificate showing parental relationship;

4) Parent(s) photo identification; and two identical passport photos.

If child has been issued a previous U.S. passport, the passport must be provided at time of application.

Passport photos

Photo service is available at the City Clerk's Office. The fee is $15.

Note: A single check cannot be used to cover both portions of the fee. Fees to the City of Bloomington may be paid by cash, check or credit card. (If paid by check, must be payable to “City of Bloomington” and must be a separate check from that written to the U.S. Department of State.) All fees to the Department of State must be paid by check or money order payable to "U.S. Department of State."
